The health assessment looks at all pillars of the Well-Architected Framework and at governance according to the Cloud Adoption Framework. In both cases you receive a report with findings and a risk rating, quick wins, a roadmap and a management summary.
In short: the Solution Architect makes sure the right thing gets built, the Cloud Consultant makes sure it gets built right. As an architect I translate your goals into a design and guard the coherence; as a consultant I configure Azure and Microsoft 365 and take care of the implementation.

No. I focus on designing, improving and securing Azure and Microsoft 365 environments, for example with a Landing Zone as code. Moving servers or mailboxes is not something I offer.
Yes. In every assessment I check whether you are ready for AI: who has access to which data, and is sensitive information labelled properly? With Microsoft Purview, sound access rules and, for Azure AI services, an AI Landing Zone, I lay the foundation. And in a roadmap we decide which AI applications really pay off.
That's the idea. Every engagement ends with a handover: documentation for administrators, an explanation per measure and, where needed, a training or workshop for your team.
